The Division does have a Lab Incident Investigation form available at

It is a 2 page form; the first page is designed to describe the incident from the witness's point of view; the second is designed to help do a safety analysis of the event. We haven't pushed it out to the world very much because we're not clear how the information collected is best used. Which is why I raised yesterday's question.
